Continued Learning for All Disciplines in Acute and Home & Community Care

This online module is designed to be followed by the RPACE: Serious Illness Conversation Guide virtual education session. The session will include a review of each of the questions in the conversation guide along with helpful tips to use in practice, as well as a simulation activity where you will use the guide with feedback from an RPACE Lead/Educator.

Coaching and Mentoring Support

Please contact your regional RPACE Lead/Educator at any time for ongoing support and joint visits.

They can provide 1:1 coaching and/or demonstrate having these conversations with your patients.

They can share just-in-time education at huddles and work with your local leadership to find ways to better incorporate this practice in your setting.

Advancing your Practice in Challenging Situations and Taking the Lead with Serious Illness Conversations

By taking the RPACE Advanced Training: A Deeper Dive into Challenging Situations, you can develop additional skills to help you navigate strong emotions and patient resistance to a Serious Illness Conversation.

The RPACE Advanced Training: Taking the Lead with Serious Illness Conversations, will use your passion for patient-centred care to build upon your mentorship skills and help foster a culture shift in your care setting.

Ongoing Learning Opportunities

Enhance your practice in having early conversations and connect with clinicians from other areas in VCH through the RPACE Community of Practice Quarterly Learning Series. Past topics have included:

  • SIC in the context of COVID-19
  • Differences between MOST, Goals of Care and SIC
  • SIC with clients with Mental Health and Substance Use Concerns
  • Language and Cultural Considerations for Goals of Care Conversations
